Camp Nexplore Waiver and Release

 

ACKNOWLEDGMENT OF RISK AND CONSENT TO PARTICIPATE

In consideration of the Participant being permitted to attend and take part in Nexplore programs and camps, I, on behalf of myself, my heirs, family members, and assignees, voluntarily release, waive, and discharge Nexplore, its employees, agents, and representatives, as well as the host school or venue, from any and all claims or liabilities for injury or property damage that may arise from or relate to the Participant’s involvement in Nexplore activities, including those resulting from the ordinary negligence of Nexplore or its personnel.

 

I understand and agree that the Participant will follow all instructions and safety rules set by Nexplore instructors, along with any posted or generally expected rules of conduct. I further certify that:

 

  • Any medical or health-related conditions that may affect the Participant’s ability to safely participate have been disclosed in the registration “Special Needs” section.

  • The Participant will immediately inform the instructor of any discomfort, health concerns, or unsafe conditions experienced during the program.

  • The Participant may discontinue participation at any time if they feel discomfort, stress, or are unable to safely continue.

    EMERGENCY MEDICAL AUTHORIZATION

    In the event of a medical emergency, I authorize Nexplore instructors to call Emergency Services (911) to assist my child. I agree to be financially responsible for any medical treatment or transportation required as a result.

     

    PHOTO AND MEDIA RELEASE

    I grant Nexplore permission to photograph or video my child during participation in its programs and camps and use such images on its website (www.nexploreusa.com) or promotional materials, without compensation. My child’s name and identity will not be disclosed.

     

    If you DO NOT consent to audio recordings, photographs, or videos of your child being used in promotional materials please also notify us via email at camps@nexploreusa.com

     

    PRIVACY NOTICE

    I understand that Nexplore’s online registration system is provided for convenience, and that Nexplore does not sell, rent, or share my personal information with third parties. I acknowledge that Nexplore is not responsible for unauthorized access to this information by third parties.
